About the Team

This friendly and well‑established team includes Social Workers and Social Care Assessors who are co‑located with District Nurses and Community Matrons, creating a truly integrated, multidisciplinary environment. The team delivers a holistic, strengths‑based and personalised service to adults aged 18+ with physical disabilities, long‑term conditions and a range of physical and mental health needs.

The team is based in East Sheen, with excellent local partnerships and easy access to GP surgeries, the library, Sheen Day Centre and the Alzheimer’s Carers Café.

Your Role as Senior Social Worker

In this senior position, your responsibilities will include:

  • Leading on complex assessments, risk management and safeguarding interventions.
  • Working collaboratively with residents, carers and multidisciplinary partners to support independence and wellbeing.
  • Applying relevant legislation—including the Care Act 2014—and ensuring all work reflects best practice and statutory requirements.
  • Overseeing budgets related to care planning, ensuring timely and accurate decisions within available resources.
  • Providing professional supervision to Social Workers and Social Care Assessors, offering guidance, oversight and quality assurance.
  • Modelling high‑quality practice and contributing to the overall leadership of the team.

This role includes hybrid working, combining office‑based work, home working and community visits.

What We’re Looking For

We are looking for a confident, knowledgeable and experienced practitioner who can demonstrate:

  • A recognised Social Work qualification and Social Work England registration.
  • Strong post‑qualification experience working with adults in a local authority setting.
  • Skilled decision‑making, strong professional curiosity and the ability to manage complex situations.
  • Excellent understanding of safeguarding duties, relevant legislation and local authority responsibilities.
  • The ability to provide effective supervision, maintain high professional standards and support the wider team.
