Are you passionate about supporting vulnerable adults and helping them achieve the outcomes that matter most in their lives? An exciting opportunity has arisen for a Social Care Assessor to join our Hospital Discharge Team, working with out-of-borough hospitals to ensure safe and timely discharges.
You’ll be part of a diverse and dedicated team, collaborating with health professionals, families, and carers to assess needs and create person-centred Care and Support Plans. This is your chance to make a real impact in your community while developing your skills in a supportive environment.

What You’ll Do

  • Carry out Care Act 2014 assessments for adults and carers, ensuring eligibility and safeguarding needs are met.
  • Identify opportunities for preventative services such as equipment, minor adaptations, care technology, or enablement.
  • Explain personal budgets and promote direct payments, ensuring clarity on charging policies.
  • Develop and review Care and Support Plans that promote independence and well-being.
  • Maintain accurate case records and share information appropriately.
  • Work collaboratively with health colleagues, voluntary services, and private providers.
  • Participate in supervision, training, and team meetings to continuously improve practice.

What We’re Looking For

  • Knowledge of the Care Act 2014, safeguarding principles, and the roles of Adult Social Care.
  • Experience working with clients/service users, ideally in a social care setting.
  • Strong communication skills – verbal, written, and IT proficiency.
  • Ability to manage your own workload effectively and work collaboratively with others.
  • Numeracy skills to support understanding of personal budgets and best value.
  • A social care qualification (e.g., NVQ II/III) or willingness to work towards one.
  • Commitment to learning through practice, supervision, and training.
